Enrollment takes place at the Student Records Department Monday through Friday. Please call 972-255-1800 for information.

To register, students entering the Universal Academy for the first time must bring:

  • Current proof of residence dated within the last 30 days
  • Birth certificate
  • Immunization record
  • Proof of guardianship or custody, when appropriate
  • Recent report card or unofficial transcript from their previous school

Pre-Kindergarten and Kindergarten

The eligible age requirements for children entering prekindergarten and kindergarten in September are:

  • Prekindergarten students must be 4 years-of-age on or before September 1.
  • Kindergarten students must be 5 years-of-age on or before September 1.

Proof of Residence
Current proofs of residence, such as the following, are required, (dated within the last 30 days):

  • Electricity bill
  • Current water bill
  • Property tax statement
  • Lease or deed

If you are unable to submit any of the above items, bring with you whatever other proof you think would tend to demonstrate that you are a resident in our school district. If Universal Academy disagrees with your claim that you are a resident, you will receive a written statement setting forth the reasons for denial, the effective date of the denial and the procedure for an appeal.

Guardianship
Students who are not residing with their natural parents are required to provide proof of guardianship.

Immunization

A student must be fully immunized against certain diseases or must present an affidavit or certificate that, for medical reasons or reasons of conscience, including a religious belief, the student will not be immunized.

The immunizations required are diphtheria, rubella (measles), rubella, mumps, tetanus, poliomyelitis (polio), hepatitis A,  hepatitis  B, vermicelli  (chicken  pox)and  meningococcal.  The campus nurse can provide information on age- appropriate doses or on an acceptable physician-validated history of illness required by the Texas Department of State Health Services. Proof of immunization may be established by personal records from a licensed physician or public health clinic with a signature or rubber-stamp validation.

The vaccine requirements apply to all students entering, attending, enrolling in, and/or transferring to the LEA. Incomplete immunizations will prevent enrollment in school or can restrict attendance once enrolled if immunizations fail to be updated according to the requirements.

For exemptions based on reasons of conscience, only official forms issued by the Texas Department of Health, Immunization Division, can be honored by the LEA. If a student should not be immunized for medical reasons, the student or parent must present a certificate signed by a U.S. licensed physician stating that, in the doctor’s opinion, the immunization required poses a significant risk to the health and well-being of the student or any member of the student’s family or household.
